• Step 1: Purchase a new Evolution* or Juice w/ Attitude System from TruckAddOns.com between 8/15/07 - 09/30/07.
• Step 2: Download and complete the claim form from www.edgeproducts.com or from TruckAddOns.com.
• Step 3: Mail the claim form, original UPC code and serial number (cut out from box), along with a COPY of your sales receipt.
• Step 4: Receive a check for $50 or $100 in the mail from Edge. Please allow for 8-12 weeks from the date the form is recieved for processing.
